Your First Visit
On your first visit you will be seen by one of our hygienist’s first. She will do a cleaning, take x-rays and possible perio chart, then you will have a complete exam by one of our doctors. During the exam we will discuss all of your questions and concerns about your dental health. If you’re having a toothache, your first visit will be with the doctor to address your immediate needs.

Patient Forms
We will have New Patient paperwork for you to complete. You can print it by clicking on the link below, fill it out and bring it with you. You can also get it when you come in and complete it before your appointment. Please arrive 15 minutes early if you need to complete paperwork when you get here.
Payment & Insurance Options
We accept cash, personal checks, Master Card, Visa, Discover, or Care Credit. Our office is a Delta Dental Provider, however, we do accept nearly all major insurance plans. Even though dental insurance is a private arrangement between you and your company, we will file your claim, handle all the paperwork, and help you get maximum insurance benefits. Ask us about our easy financing options such as Care Credit, an interest-free, flexible monthly payment plan you can apply for right in our office.
Please make sure to bring your dental insurance information with you. Payment is due at time of service. We are a Delta Dental provider; however we accept and process other dental insurance. Forms of payment are check, cash, Master Card, Visa, Discover or Care Credit.
